- Current subscribers should complete the Member Information Form and the Open Enrollment Election Form
- If an existing group has new enrollee’s they should complete the enrollment form attached to the enrollment packet
- Anyone wishing to make changes should complete the change form
- Anyone wishing to choose a Dental Plan needs to choose Option A or B
